Nursing in Crisis: How Hospitals are Dealing With the Shortage

The Current State of Nursing

Healthcare delivery has undergone critical change over the last several years, and the nursing profession has changed along with it. The end result: A reduction in the supply of qualified, experienced nurses. Those who remain are working under conditions of heavier case loads, technological complexity and increased acuity of patients. A condition which once warranted a lengthy in-patient stay is now treated on an out-patient basis, largely because of managed care requirements.

According to Debbie Pickup, Director of Human Resources at Southcoast Healthcare System and a former R.N., "The nurse's role of patient care is the same, but the importance has escalated. Since hospital stays are shorter, a patient who undergoes surgery used to stay for one week, but now only stays two to three days. Nurses must assist the patient to bring him or her to a functioning level and put together a plan to support the patient and assure quality care, while discharging sooner." According to Barbara Munro, Dean of Nursing at Boston College, "Nurses have said that the problem has been the working conditions - too few nurses, not enough support, an uncertain future, and forced overtime."

The role of patient families has also changed in the last decade. "Both patients and their families are more educated and involved in the process," according to David DeJesus, Vice President of Human Resources at Southcoast Healthcare System. "Families used to trust the care of the patient to physicians and nurses; now it's common that family members stay 24 hours a day, ask questions about the care, and put more demands on the nurses."

Reasons for the Shortage

The current shortage in qualified, experienced nurses is not expected to improve over the next several years. In fact, the majority of senior level nurses and nurse managers are reaching retirement age. As they exit the profession, fewer nurses will be available or willing to assume a management role. The current climate of understaffing, 24-hour shifts, increased management responsibility, and increased acuity of patients often leaves younger nurses reluctant to fill the gap.

Dean Munro sees the problem continuing. "Most nurses are around 45 years old, white, suburban women, 80% of whom are currently working full-time. As they retire, they often leave because they're burnt out; unfortunately, there are simply not enough nurses to replace them."

Only a few years ago, industry analysts were stating that there was an over supply of nurses - not so anymore. Why aren't there enough nurses to replace those who are retiring or leaving the profession? Hiring managers are finding that younger Baby Boomers and Generation Xers are no longer viewing nursing as a viable profession because of the difficult work environment combined with a desire for more control, quality and balance in their lives. In addition, women have more choices of fields to enter. As one 20-year nursing veteran put it, "There are simply more options open to bright women interested in healthcare; before there was only nursing - now there is 'medicine'. I recently took my daughter to work one day, but frankly, I'm not encouraging her to become a nurse. The working conditions are horrendous, the responsibility is huge, patients are acute, and the workload index is high."

According to Dean Munro, the national level of enrollment for nursing is down, although applicants to Boston College have risen. Of 20,000 applicants, only the upper echelon (about 2,000) is accepted. Boston College, Yale, and other academic institutions are forming "accelerated programs" to attract those who wish to change careers. Accelerated programs enroll people who already hold bachelor's degrees in other fields into a one-year intensive nursing program or a two-year master's program. The average age range for program applicants is 25 to 35 years old.

Debbie Pickup from Southcoast Healthcare System witnessed an increase in the average age of incoming nurses. "The age of new nurses used to be around 21 or 22; now graduates have varied ages, often in the mid-30's. For many of them, nursing is a second career. They enter into a more acute clinical environment, where patients are sicker and more complex. Although schools have changed the curriculum, entry level grads have only basic skills - there is a great gap in the readiness of the individual."

How Hospitals Attract and Retain Nurses

Hospitals must now compete with one another to attract and retain nurses in a very competitive market. Southcoast's DeJesus notes that some hospitals are using preceptor programs to ease the transition of new graduates into high-paced, technologically complex clinical settings. "Since new grads need more clinical experience, many hospitals create a professional development transition program. In addition to a week-long orientation, this program combines classroom time, and practical time with an experienced staff member for six months. This mentor relationship helps with a gradual introduction. Preceptors are trained and compensated. Mentors are often volunteers. It is a supportive transition that has received a very positive response."

Hospitals have had to be flexible in their approach to overcome the shortage. Use of travel nurses, for example, fills gaps whenever needed. Many nurses, particularly older nurses, often work "per diem" to keep working without the pressure of a full-time commitment. During a shortage in the early 80's, some Boston-area hospitals even recruited nurses from other countries, such as Ireland. Other approaches include sign-on bonuses worth several thousands of dollars, referral bonuses, and flexible hours. Flexibility of hours may allow nurses to work three 16-hour days, for example, with the next four days off. For women with children, a flexible schedule can be very important.

Southcoast's Debbie Pickup adds, "Most hospitals are now focusing on retention. It is becoming critical to pay attention to all factors that are important to RNs in the clinical setting: having a voice and the ability to have some control over the practice of nursing. For example, a unit council meeting takes place on a regular basis and identifies issues and concerns to patient care. Years ago the nurse manager addressed this issue. Now TQM requires that more people are included in the problem solving and decision-making. Staff members are more involved in operational issues. Although some individuals still prefer that management fix all the problems, for the most part it works quite well."

Pickup added, "The professional relationship between the nurses and physicians plays a factor on retention of nurses. If there is a good relationship, one that is consultative, respectful, and supportive, then job satisfaction is increased. If, on the other hand, the relationship is not positive, there is a higher turnover in the nursing staff."

Dean Munro strongly suggests that hospitals work closely with the nurses to determine their needs and establish their value to the organization. "Perhaps the best ICU ever created was when nurses worked with the architects to design the unit. They designed a better unit simply by talking with the nurses."

Her advice to hospital management: "Work with the nurses to see what is needed to improve job satisfaction. The number one concern is not always salary - it has to do with improving working conditions, being recognized for what they do, and obtaining the opportunity to learn new things. Nurses need to get a sense that they have done a good job for their patients. Listen to the nurses. Empower them. Make them part of the process. That process will be improved and so will the quality of patient care."

Business and Market Overview of Brunei

ECONOMY. Brunei's economy is dependent on oil and gas and is the third largest producer of crude oil in Southeast Asia after Indonesia and Malaysia. Brunei is also the world's fourth largest producer of natural gas. Brunei's current oil and gas reserves are sufficient at least until 2015. Thus, Brunei's government has used its oil wealth for investments outside the country for future generations. Furthermore, the government seeks to develop the country's economy beyond on oil and gas but with little success.
Brunei's GDP was US$5.2 billion with a GDP per capita of US$13,879 in 2004. The economy grew at an average GDP growth of 3.0% annually from 2000 to 2004 driven mainly by Brunei's export of oil and gas and therefore dependent by world oil and gas prices. Inflation was less than 1.5% in 2000-2001, experience deflation in 2002-2003 but inflation eventually crept at 0.9% in 2004. The government is Brunei's largest employer and many of its citizens prefer to work with the government. The country experienced increasing unemployment from 2002 to 2004 but remained below 5.0%.
The industrial sector (mainly oil and gas related activities) contributed towards 56.1% of Brunei's GDP in 2004. The service sector contributed towards 40.3% while the agriculture sector contributed only 3.6% during the period. Main industries are petroleum, petroleum refining, liquefied natural gas and construction. Major agriculture products include rice, vegetables, fruits, chicken and eggs.

DEMOGRAPHY. Brunei has a small population of slightly more than 370 thousand. Brunei Malays are the largest ethnic group and account for nearly 70% of population followed by Chinese accounting for 15%. Others include indigenous people and immigrants who have settled in the country. Islam is the official religion of the country and 70% of the population practice the Muslim faith. Other religions include Buddhism, Christianity and indigenous practices. The official language is Malay while Brunei's Chinese community often used the Chinese language within the community. The population is generally proficient in English since schools teach the language and used in higher education, business and the sciences.
Three quarters or 75% of the population live in the urban areas and mostly work in government services, oil and gas industry, wholesale and retail trade and construction. Major urban areas include the nation's capital Bandar Seri Begawan, Muara, Tutong, Seria and Kuala Belait.
Poverty is practically non-existent in the oil rich nation of Brunei. Brunei's GDP per capita is half of Singapore but based on purchasing power parity (PPP) it is slightly less than Singapore. Nearly 70% of the households belong to the middle or high-income categories while the remaining 30% in the lower-income category.

INFRASTRUCTURE. Telecommunication services within the country well developed while reliability of services outside from Brunei is good. Internet access is available throughout many parts of the country but broadband services are limited. Towns well connected by roads and crosses the border into East Malaysia. Country served by single international airport at Bandar Seri Begawan.

INTERNATIONAL TRADE. Major trading partners include Japan, South Korea, Australia, US, Thailand, Indonesia, China, Singapore and Malaysia. Much of the imports from Singapore are Singapore's re-exports from other countries. Major exports include crude oil, natural gas, refined petroleum products. Major imports include machineries and equipments, vehicles and vehicle parts, consumer goods, foods, construction materials and chemicals.

CONSUMER USAGE OF TECHNOLOGY. Nearly all homes in Brunei have fixed-line telephones and the penetration of mobile phones by population was 40% in 2004. Brunei's general population have the financial means to install computers in their homes but the penetration in homes is low at 20%. Penetration of internet users is also low at 9% of the population or 34,000 users. Nevertheless, nearly all homes in Brunei have televisions and refrigerators.

RETAIL MARKET. Marketers into Southeast Asia often neglect Brunei as a potential market because of its small consumer population. However, the country has the second highest GDP per capita in the region after Singapore and depends on imports for nearly all of its consumer goods and foods. The estimated value of Brunei's retail market in 2004 was US$390 million in 2004 of which foods accounted for nearly US$280 million. The "mom and pop" stores and mini markets dominate the retail industry alongside a few department stores and supermarkets. Consumers in Brunei often shop cross the border into Malaysia for wider choices of consumer goods.

FOOD CULTURE. Foods eaten by the Malays tend to be rice with spicy meat and vegetable dishes. However, the people of Brunei are accustomed to Indian foods due to the numerous small Indian eateries across the country. Thus, homes often serve fish, chicken or beef curry dishes. Popular food service establishments include Chinese, Indonesian, Indian, Thai and Japanese restaurants but interestingly few Malay restaurants. Among the younger generation, many are accustomed to western style foods served by the fast food outlets and bakeries.

Real estate sales training is perhaps the most important training in which a real estate professional can participate. Of course, in every state and the District of Columbia a real estate agent or broker must be licensed but without the ability to sell, the real estate license serves no purpose.

According to estimates from the National Association of Realtors, there are about two million licensed real estate agents in the United States. It’s a relatively easy field to join. The licensing requirements, although different for each state are not that stringent. Although you can get a bachelors degree, it is not required to procure a real estate license.

Maybe because it is so easy to jump into a real estate sale explains the large number of licensed real estate agents and brokers across the country. Of course, the turnover rate is high too. By some estimates, as high as 80 percent. Some estimates are equally as high for the percentage of real estate agents who quit in the first year.

There are seminars and classes and books that offer sales training. Some large realtors provide much of the sales training for their real estate agents. Others leave the agents to fend for themselves.

Real estate sales training is not unlike any other kind of sales training. The same rules apply. You must know your product in order to sell it. For a real estate agent this translates into knowing the community, the housing market, market trends, economic facts and figures, what is currently selling and what isn’t. It also means knowing more than a little about loans and mortgages. There are far more options today for buying and selling property than there used to be and not keeping up with the industry is the fastest way to sales suicide.

Sales is always about communication. That usually entails listening more than talking. You have to know what your client’s need is before you can fill it and the only way you will know this is effective listening. That’s why the Dale Carnegie sales training has always been so effective. It teaches interpersonal skills and communication skills. You can know everything there is to know about the real estate profession but if you can’t interact effectively with people, you will likely not be successful at real estate sales or any other kind of sales.

It’s not enough to just be a nice guy though. That’s one reason it’s imperative to have sales training. Having a real estate license is not enough that just means you have a grasp of the basics and you’re legal. Knowing your product isn’t enough either and you can be the nicest person in the world and not be able to sell a dime for a nickel. Remember Gomer Pyle? Can you imagine buying anything, let alone a house or other real property from him? Sure, he’s a nice guy but how confident would you be in his ability to sell you the best house for the best money and make sure everything went smoothly?

Learning by example is often the best way to learn. Lest one should think that education administration or educational management is simply an industry term for business administration, the requirements for most positions entail academic management specific training or experience. Education on the whole is funded through tax dollars, yet students fees also play a part. Additionally, the intricacies of managing an educational institution are far more industry specific than with most businesses. Managing the sales of widgets may not be far removed from managing the delivery of widgets, but managing education is another thing entirely.

In addition to the bests interests of the student population, administrators must deal with ever-increasing class sizes and ever-dwindling budgets. Though education has received more attention in the form of tax dollars and federal initiatives over the last decade and a half, the educational system is still operating less effectively and producing fewer high school graduates per students enrolled than in the 1980’s. This trend seems likely to continue unless education becomes a higher priority at the federal level.

Still, government mandates and money alone won’t remedy these problems, which is why societal changes to the way we view education are necessary. For education administrators who are most accountable for the performance of their institutions, these changes can’t come soon enough. When the contributions of teachers and other educators are better recognized through higher salaries and increased job stability, administrators will be able to focus less on the appropriation of funds and more on ensuring that the schools they preside over are meeting the highest academic standards.

As business owners, you are all aware of the recent vote to raise the state minimum wage to $6.75 per hour, per Arizona voter approval of Proposition 202. In addition, the new law mandates annual wage adjustments according to increases in the cost of living (CPI). As an Arizona Human Resources Consultant, I am working with clients to inform and educate on the new regulations and impact on businesses in the community.

Most businesses will be required to comply with the new state statue, however there are limited exceptions, including any person who is employed by a parent or sibling, "casual" babysitters, the Arizona state government, the federal government, employees who regularly receive tips (may be paid a minimum of 3.75/hour), and businesses that have less than $500,000.00 in gross annual revenue and that are exempt from the state minimum wage law.

OVERTIME
The increase in wages will obviously effect overtime pay, whereas one who earns $6.75 per hour, will earn $10.13 per hour for hours worked over 40 in a workweek This change will effect the ways in which employers manage overtime of non-exempt staff. Perhaps this encourages an emphasis on work/life balance and reminds us the 40-hour workweek as originally defined by the Fair Labor Standards Act of 1938.

TIPPED EMPLOYEES
Currently, tipped employees are paid a minimum of $2.13 per hour plus tips, which must equate to at least minimum wage in a workweek. When reported wages are less than minimum wage, the employer is responsible for adjusting the employee's pay accordingly. Under the new statute, employers may pay $3.00 per hour less than the state minimum wage ($3.75 per hour). Arizona Restaurant Association was a big advocate in fighting against the raise in minimum wage hike.

RECORD KEEPING
Proposition 202 requires new measures in record keeping requirements. The Equal Pay Act and the Fair Labor Standards Act both require retention of payroll record information for three years; the new provisions necessitate retaining records for four years.

IMPACT ON BUSINESSES
Offsetting costs may be possible by passing it along to consumer. Some business may need to assess their current workforce and reductions may be in order. Businesses may consider automating processes that may result in cost savings. Outsourcing will likely become a significant cost saving measure.

IMPACT ON WORKFORCE
As low wage ($5.15 to $6.75 per hour) earners will be impacted, thusly so will workers receiving wages in excess of $6.75 per hour. Known as the "ripple effect," Theoretically, all workers should experience a positive impact with regard their pay. Negative impacts may also arise from loss of jobs, higher income taxes and increased cost of products.

Since the passage of Proposition 202, there has been much to be said on both the positive and negative impact of raising the state minimum wage. Nevertheless, Arizona business owners and their employees are likely to face significant changes in the coming months.

Are you sick and tired of all the great do-nothing-and-become-rich opportunities? If you are anything like me, then you probably are. I am just so tired of people emailing me with the next hot opportunity, where you literally don't need to do anything to earn a great income. Well and if I decide to take a closer look what it is all about, its just another MLM scheme.

Now don't get me wrong, I have nothing against MLM but I have something against people who say that I don't need to do anything to make money. That's just a lie and how could you ever work with a person who lied to you since the day you have met them?

Being so tired of all this, I decided to look for online opportunities that really required no work and I have stumbled over one last year. Since them I have earned great money with these opportunities and so can you and it really doesn't require any real work besides doing some due diligence.

So what are these opportunities? They are called HYIPs. Maybe you have never heard of HYIPs, so I will give you a short introduction:

HYIP stands for High Yield Investment Program and I think that says it all. Basically these HYIPs are investment opportunities which promise really high returns. Returns that are sometimes much greater than 20 percent monthly interest. Yes, you have read correctly. I said 20 percent per month or even more. Now when you just start to dive into the HYIP industry, you will shortly realize that many programs offer interests that are even higher than 20 percent. Some programs offer 100 percent per day or similar ridiculous returns. If you have read that far, you might think this all must be a scam right?

Well, you are right but not completely. Honestly the number of scams is huge in this industry and I would say it´s about 99 percent but what about the other 1 percent?

Those are real investment opportunities. Opportunities that provide the chance to invest in a fund that is day-traded in various markets, mostly the forex (foreign currency exchange) but also commodities, futures or stocks. The foreign currency exchange market can be one of the most lucrative markets to trade but also one of the most dangerous because of the high volatility and access to high leverage of trading equity.

Never the less good traders can achieve 20 percent or more monthly returns when trading the forex. Apart from the opinion of many investment experts this can also be done on a consistent basis, which does not mean that you will always achieve a 20 percent interest but there are programs out there that produce an average of 20 percent per month over the course of a couple of years.

How can you know that the HYIP you have chosen really does trade? Well, you can´t or at least that´s very hard to find out. Some legitimate HYIPs let well-known people from the HYIP industry take a look at their live trading accounts and I had the privilege to analyze some myself. Other than that there is not much proof so what do you do?

I personally favor managed accounts. Managed accounts are a little bit higher investments, starting from USD 5000 but they are well worth it for two reasons.

1.) Nobody can steal your money so it's a no-fraud-opportunity and 2.) You can check your daily performance because you have live access to your account with the brokerage.

Now do some calculation on your own what a 5, 10 or 20 percent monthly interest could mean to you. I will give you an example: If you invest 1000 USD today and let it compound for 2 years with an average of 15 percent monthly interest, you will end up with over 28,000 USD or a monthly income of 4300 USD. Now I don't know how much you need to live but that is a nice residual income, which might pay some of your bills. Always remember, be careful where you invest and if you can afford it, go for managed accounts.
